265-Hour I-ACT Professional CHT Training Enrollment Agreement

Prerequisites for Professional Level Course:

• Must have a high school diploma/GED, or equivalent
• Must have a current CPR card
• Each new student should have received 1-3 colonics prior to entering training

Length of Course: 265 hour course is divided into 200 hours of online coursework and 65 hours in-person training at our facility.

Course Length: 200 hours of online coursework is self-paced with a max time limit of 6 months to complete. The 65 hour in-person training can be completed in no less than 7 days at our facility. In-person training may begin once you have completed 100 hours of the online coursework. In-person training dates will be coordinated between student and instructor.

Program Starts: Ongoing open enrollment.

Payment Information

1. Tuition Amount

A. Tuition for the course is $5700.00, which includes the application fee of $900.00 and program cost of $4800.00. Application/administrative fee is non-refundable.

2. Other costs

A. Travel, food, and lodging expenses.

Payment Schedule

A. The application fee of $900.00 is requested with submission of the application form and enrollment contract. This fee includes administrative fees for online training, I-ACT and NBCHT application fees. The application fee is nonrefundable.

B. Both the application and enrollment contract should be completed and returned with payment.

C. The remaining balance of $4800.00 must be paid in full prior to beginning the online training course unless approved by Jen Stanley.

Late Charges and Cost of Collection

An additional $35.00 fee will be charged on any overdraft or inability to process fees due and payable to From Scratch Wellness Services.

Refund Policy

A. If an applicant is not accepted having made an initial payment, all monies except the administrative fee of $350.00 will be refunded.

B. An applicant who provides written notice of cancellation within two (2) days, excluding Saturday, Sunday, and federal or state holidays of signing an enrollment contract is entitled to a refund of all monies paid minus the application fee of $900.00.

C. If the student does not complete the 200 hours of online training within the 6 month time limit, no refund of tuition will be provided.

D. If the student begins the online training and withdraws before completing the first 100 hours of online training, they will receive a refund of 30% of the full tuition.

E. If the student withdraws after having begun the in person training no refund of tuition will be provided.

F. If From Scratch Wellness Services cancels the in person training less than 14 days before start date of the in person training, a refund of 65% of full tuition will be provided to the student.

G. Students must notify Jen Stanley of From Scratch Wellness in writing delivered in person, mail or by electronic mail if they need to cancel out of the program. The notice must be signed and dated by the student.

H. Refunds are mailed within 30 days of the date of notification.

Procedure for Withdrawal

A. Withdrawal requests must be written, dated and signed by student

B. Student may be offered an opportunity to return at a later date

Dismissal Policy

A. Failure to satisfactorily complete the academic or internship aspects of the training, non-payment of tuition, unethical behavior, or failure to comply with training guidelines will result in dismissal from the program and no entitlement to refund of tuition.

Professional Standards Certification

Certification standards vary greatly in different jurisdictions. Our program meets or exceeds the standards set forth by the International Association for Colon Hydrotherapy, the largest professional organization overseeing standards in the field of Colon Hydrotherapy.

Job Placement Disclaimer

This program does not guarantee job placement to graduates upon course completion or upon graduation. Therefore I acknowledge that job placement and compensation cannot be guaranteed by From Scratch Wellness Services.

Notice to Applicant

To fulfill From Scratch Wellness Services intention of providing high quality education, this program may from time to time modify content or schedule in order to improve curriculum and/or reflect changes in national standards. From Scratch Wellness Services reserves the right to cancel the course at any time. In the event of cancellation a refund will be issued in accordance with the refund policy set forth in this contract.

Training will be performed on a FDA certified closed colonic unit, this is not manufacturer training, this is the 265 hour I-ACT Professional Certification training course.
